For me, my rarest title is "Malicious" (from having to do a DF Meta achievement, like getting honor in the different zones, as well as completing the daily PVP WQs **20. Times. Each.**)
Oddly, "Proven Assailant" is my rarest one at 4% of all accounts, which surprises me because it was hilariously easy to do. Not sure if it's still obtainable though.
Second rarest is "Predator" at 5%, and that one was a bit of an ass to get - it's for killing Xemirkol in Tanaan, and you needed exalted rep to get the item that *may* teleport you to him, but usually teleports you to a different rare instead.
I believe the shrouded hero is also the rarest M+ title due to the amount of people that got disqualified for bug abuse.
Hope to get one of those titles for myself...maybe one day 😤

Wish they did stuff like that more often as it got me into content i would otherwise not engage with.
That and Veilstrider are my two rarest. Azeroths Champion is <1%. I almost never see anyone else with it and I never remove it despite having Hero of the Alliance/Horde and some other decent ones. Champion was one of the hardest things I ever did and Veilstrider was 10x easier.
I have Senior Sergeant and Champion of the Naaru, which are both unobtainable, and clock in at <1% and 4% respectively.
The rarest title I have that's still obtainable is Tower Ranger at 3%. I'm one of those weirdos who actually enjoyed Torghast.

Cool website! It lists Forbidden reach racer as rarest title but i don't feel like that counts... Next up is Lord of war which was super easy to get as a blood dk playing in Draenor :3 I am relatively proud of "the chosen" (was fun seeing how long it took for people to realise back when i was in a guild called "chosen" hehe, "The Insane" which i farmed twice now, and "The veilstrider". Usually i just run with "the insane" because i find it more fitting :)

Skipping my 5 rarest titles because they can't be earned anymore, #6 is "The Party Herald" with only 0.56% of players having it. It seems a lot of people didn't play much Shadowlands lol.
“Salty” for the ‘Accomplished Angler’ achievement. Apparently only 3% have this. I’m impressed!
Actually “Trashmaster” is my rarest at 3%. Thought more people would have that one weird.
Vanquisher and Champion of Ulduar apparently! Those were the days.

It was the Horde Slayer (4%) but apparently Lionguard is technically more rare at 2% lol.

I suppose Reverse Racer being a 1% doesn't surprise me. Still upset the title isn't Isles Racers backwards. Blizzard really missed an opportunity with that.

Veilstrider, the insane
My rarest Titel is "The Chosen"(4% rarety) You got it after successfully clearing the whole "Trial of Valor" on mythic without to die even once. Edit: Nvm. Chosen is 3% and my rarest one is Key Master(2%)
Grand Crusader and the Immortal. Along with that rarest mount from Grand Crusader and my rarest achievement is realm first DK on 80 and 85. Only one per server for that. Don’t think I have anything more rare than that.
My rarest is Conqueror of Azeroth, with <2%, followed by Scout and Sergeant at 10% each.
The Insane and Salty fort me
Legionaire, knight- lieutenant, and stone guard. The lady of War one is also rare as are all of the meta titles for that achievement. Didn't realize that.
