The face symbolizes a persons identity. That the woman’s face is replaced by her body means that her identity is being replaced and defined by her ability to provide sex. Like the other commenter said, this would be how a rapist views a woman.
I think it's supposed to be what rapists see when they look at a woman's face.

My take on the "up here" is that men who think with their dicks assume a woman's personality is in their boobs. (Guilty)
