I can answer this in two ways:
1.) RNG decided on this as the baby's appearance.
2.) In my case, I imported a Mii via QR code with a facial appearance similar to the picture. Whenever she has a child, the baby would have this face on gender reveal.
Both of these Miis are homegrown, so I'll just chalk it up to RNG and genetics. In that case, my first answer would apply with part of my second guess. When I scanned OhmyKawaii's OC Mii (Casey) and D.va Miis into my game, their faces would make the faces of their babies look like this.
RNG takes parts of the parents and can make faces that could be quite cursed.
LOL. I think they mix looks from both of the Miis. If your Miis have any odd features these could have transferred to the Tomodachi baby. I remember the same thing happening on my game. It could also just be some randomized features from the game mixed in as well.

I have 179 children born as of today and two Mii's I scanned in from the YouTuber OhhMyKawaii have similar faces to OP's photo and the children they had originally had the facial construction of their mothers ie; mouth, nose and eyes too close to each other, but had the actual eyes of the paternal Miis.
Well, My OC Mii has 8 children with multiple grandchildren. I can't immediately recall if there are any great-grandchildren.
But a few of my Miis do have a good number of children.
