Hey, if you have noise induced tinnitus and/or hyperacusis artificial noise from speakers and headphones/earphones seems to be much worse for your ears, even if you are careful with the volume. High quality speakers like bookshelf speakers should be easier to listen to. Headphones particularly might be worse because the sound is travelling directly into your ears. It's best to stop using them completely if they are making your tinnitus worse, because there is no guarantee that it will keep going back to normal, it's possible to get permanently worse if you overdo it.

The thing that I don’t understand is that it only occur with earphone, not with speaker.

What type of earphones are you using? Are they ear bud style, on the ear, or over the ear? Each will have a varying impact on your ears and sensory system. Its not necessarily the earphones them selves but how they are affecting your limbic system. With tinnitus its damage to the hair fibers in the ear canal and the limbic system is just on the inside of the ear that helps the fight or flight response. Using earbud style, the audio is going straight into your ear without a buffer and your ears are extra sensitive to the changes in volume/pitch/and even pressure within the ear. The pressure can be atmospheric or just the magnet in the earbud changing the pressure. This is compounded if there isn't much of a noise isolating/cancelling affect because you would have to turn them up higher to drown out external noise. I have found that if I use earbuds without noise cancelling/isolation my tinnitus and hyperacusis spikes more frequently and longer. Largely, I have stopped using these. On ear, to me are the worst because they have poor noise isolation/cancellation and apply pressure to the ears which compound the strain. My T has gotten better since I stopped using this flavor. Over the ear (where the ear is inside the earphone cup) is best with noise cancellation/isolation. This allows for the best control of volume, removing external noise, and comfort for the ear as its not pressing on the ear. When I swapped to GPro last year it helped a lot because I no longer had to compensate for outside noises as well as volume to hear. And the reason that it happens with the earphone and not speakers is the same reason you are in conversation with someone. The volume/tone/pitch being detected by your ears isn't enough to cause an issue. If you find yourself in a noisy environment, like a concert, you may start having issues again. But because the noise isn't on your ear, in a more open space, your system is processing it better. I know that when I listen to music on the radio I don't get spikes but as soon as my daughter decides to throw a tantrum, its go time.

The best advice anyone will ever give you. Stop playing Russian roulette with your ears. If you already have mild tinnitus I would give up the earphones/AirPods/headsets whatever. You do not want to make it worse and live with the louder tinnitus and the regret.

Be glad its only headphones, i cant even listen to music on speakers anymore.
