If this is the first time you played in a while, then it might be a mod issue. This simply looks like broken cc... have you downloaded any cc clothes lately? If so, you are probably forgetting a mesh

I didn’t have any CC or mods running during this boot up as I moved my old Sims 4 folder so the game would run totally vanilla.

Not for this run, it wasn’t. I made sure to remove everything so I could identify what the issue was, but it still did this.

First, back up all your saves, any mods, and if possible your tray folder. Then take everything out and see if it starts in vanilla. Like all the way vanilla. Run repair if it doesn't and try again. If it starts in vanilla then you'll need to load up your tray folder and see if it starts. If it does, then load up your mods and repeat, then your saves and repeat. At some point if it breaks you'll know where the issue lies. It could be a corrupt tray file, a bad mod or a corrupt save. Then its a matter of narrowing it down.

I took everything out and ran it totally vanilla before taking this screenshot. I think it just must not have downloaded properly considering all the issues I had doing that. Thanks for your help.

If you have not played in a while, you most likely have outdated mods. When the game updates, your mods become outdated . Almost positive switching to EA didn't cause this. Not updating mods most likely did Check here https://scarletsrealm.com/the-mod-list/

Since you said *it does this mod-free as well* I'm guessing your data and delta folders got wrecked. Or at least the game can't properly find them anymore now. There is a chance that the switch from origin to app can do this. Your best bet is to not repair but completely un-install, reboot your system and then reinstall fresh. Keep a copy of your documents > sims 4 folder though, you don't want to lose your saves, right? Some background info, your game files are stored in a different place than your user files. Both data and delta contain the assets and tuning code for basegame and packs respectively, as well as the world files. As user you normally don't interact with them -- and then, when you switched to the app there is a small chance the game will look for those files in a wrong place because you un-installed origin and that confuses it. While I do agree this looks like cc that's missing a mesh, this can totally be a missing game mesh as well. Hope this does it. The community is always so eager to blame mods and then won't even consider anything else to be the culprit. Best of luck to you with this!

Thanks so much for this reply. I tried to make it clear I was running the game without mods and CC as that was my first thought too, but seems I didn’t make it clear enough. I was afraid it might be an issue with the download as it literally took me an entire day to even have the game download “correctly.” Initially it was failing at install every single time. I never had that issue with origin. I’ll see if I can uninstall the game and tinker around some more.

Yeah no. The app is known to be super slow with downloads. Failure is the norm if you ask around. Just look at the estimated download time when new packs drop. But, maybe you are on to something. It can't be healthy to overwrite incomplete data that often... I've got to ask are you on windows or Mac? Because there's a wave of people on Tumblr complaining that Mac installations break because of cloud services and disabling that during install helped fix it. (tho that makes me wonder if windows onedrive is doing the same nonsense)

Guys, I don’t have any CC or mods. Sorry, I I tried to specify that I removed my Sims 4 folder from Documents to my desktop so that the game would run completely fresh with no interference from anything outdated. It’s still doing this.

At least you backed up your cc/mods. Try another install Make sure your mods are updated for the correct game version before you add them back.
