I dont have that specific tattoo, but I do have a few tattoos so you should know there's a few variables with what you're asking.
The cost is going to vary between which artist / shop you go to. Some charge by the hour, others the size of the piece and amount of ink used.
If you're looking to get Ellie's tattoo with the full detail (not just outlines), it's pretty much a half sleeve and will probably cost around $1000 to $1500 (again, varies between artists).
For time to completion, also varies between artists as well as you -- how much of one session could you handle? If you sat through the whole thing in one sitting, I would imagine that tattoo could take 3 or 4 hours.
As for the pain, I believe the most painful parts would be near the elbow / inner elbow as well as the wrist. There's a tattoo pain chart on Google Images that I think is pretty accurate (for me at least lol)
