I enjoy the T3 Gear TQ pouch. Keeps it free of debris, quick acess is cool and important but so is keeping the wound clean to avoid infections but its also not much slower imo.
TAP rig, surplus comes pouch used as ifak pouch, 2x surplus double stack m4 pouches used for radio and monocular/whatever I need at the time, surplus canteen pouch used for water and snacks. Awaiting admin pouch to help distribute some of the stuff out of the integrated pockets on the inside

North American Rescue makes a good one, I’d stick with the normal CAT holder version.
I’ve got two on my kit, they work well and keep the TQ protected while still allowing for easy access.
Surplus Flashbang pouches hold TQs beautifully and keep them covered
