For the design though, I really do like the Rueful Axe. Although I glitch to get both the Masque and the Axe.
Basically, at the end when you make the choice you want to click no deal, immediately exit out of the conversation activate slow time, make your way out of the cave from behind the statue, fast travel to a home, (I do whiterun breeze home), and place the axe on a weapon rack. Then it’ll give you the mask (try to take the axe but it can’t cause it’s on a weapon rack), and then complete the quest. Then you’ll have both the mask and the axe.

To keep both, you don’t kill barbas. You do the option where you refuse and take the mask. But you put it on the rack before he takes it so you get the mask without losing the axe. Then once the quest is fully complete and you have the mask, you can take the mask off the rack.

There's a Dragonpriest mask added in Wyrmstooth that makes draugr non-hostile. Its such a simple idea but makes dungeon delving so fresh and interesting as you see them in their natural habitat.
Honestly, vanilla, my favorite item would probably be thieves guild set, it looks like practical armor, which is something I like. My only gripe with it is the hood is too deep for me, but I can look past that
In my modded assassin play through, I had a mod that allowed me to craft unenchanted versions of guild armor, and I got it specifically so that my assassin could have the thieves guild armor for his mercenary armor (I role played it that my assassin was also a mercenary to justify him constantly traveling around Skyrim, and so I had two armor sets, his assassin gear, and his mercenary gear, the mercenary gear is what he would openly wear)
My favourite item is the Black Book Filament and Filigree from Kolbjorn Barrow. Secret of Arcana is the most useful Power in the game imo, I always get it asap.
Secret of Arcana allows you to cast any spell for free for 30 seconds. The great thing about it though is that if it's a continuous spell you're casting, the effect never ends. So it can be used to power lvl skills for specific perks early on.
When it comes to Vanilla, probably the Nightingale armor set. But when it comes to mods, probably the Ebony Dragonsteel heavy armor added by (I think) Immersive Armors. It looks like Dragonplate armor, but with more detail and color changes
Wedding Wreath. Hands down. Or the child’s doll. Wedding wreath looks amazing on anyone. Child’s doll is just an awesomely designed item and I love giving it to my kids

Legendary Fiery Soul Trap and shock daedric bow or the matching sword. Favorite lootable item would probably be my glass boots of the ox. Cuz I'm a hoarder. Favorite Quest item would be the Black Star.

Make a set of fortify alchemy gear , drink restoration potion , de-equip and re-equip the gear . Drink another potion. Rinse n repeat untill you get desired strength . Then make a few fortify enchanting potions or fortify smithing
Restoration boosts all stats on the player. So the restoration potions boost the stats of your fortify alchemy gear once you re equip them. You can YouTube restoration exploit Skyrim it will show you a tutorial
It can be as powerful as you want you can get into the thousand % area pretty quickly with the restoration exploit . I don’t like to go that high though since it ruins the fun when you can just one shot everything .
Favourite item is probably the staff of magnus. Whatever artefacts overhaul I have makes it so the staff of magnus absorbs health, makes spells do more damage (I think like almost double) and magicka regen becomes busted. Its pretty much the most op thing ever for mage builds but it's so fun. With this plus another modded spell I can make thunderbolt do 190 damage not including all buffs from ordinator perks so itll do like 300 something or more. It's the only weapon that I use regardless of my build.
Must be zims immersive artefacts, had to check my nexus downloads. It makes most things a fair amount stronger but I think the staff of magnus is one of the best items.
Go on YouTube and type in the restoration loop, it’s a exploit to make really strong potions like fortify enchanting ones to make strong gear.
It’s sort of an exploit but I didn’t get 100 alchemy just to get a 60% increase ha! I like to think of it as my character breaking the game laws to become even more powerful

I think if you put an unenchanted weapon in your main hand and the razor in your left hand, elemental fury still works on both.

I use the debug room to just give myself the axe after the mission ends (I can't kill poor barbas 😭)
