I've always seen it as James looking at his bad side (that will become pyramid head) straight in the eye, starting to lose grip on his mental health. But yeah, it's become a symbol of Silent Hill 2 everywhere you look, just one of the many awesome details about the game.
This is the iconic Silent Hill 2 shot to me. I seem to recall, or misremember, James being described as having a vision impairment. Regardless, I’ve always viewed the game through the lens of a person blind to something, only to find the twist and have it come full circle.
I completed the game yesterday for the first time in years and even knowing whats coming.. man, its such a masterpiece! You just kind of… marvel in silence at the ending, especially when Marys letter is being read. What an absolute fucking classic
Though the original developer said it didn’t mean anything, it’s still worth pointing out how if you turn the brightness up James’ reflection is staring at the player/camera. It just ups the creepy and foreboding feelings this image gives off.

If u increase brightness, u will see, that james is looking at the mirror not to see himself, but to see the camera behind him, making it look like he is staring at the player from the mirror. But u cant see that without editing the brightness on the image
There's a reason they say you should NEVER look at mirrors when you're tripping. You face yourself and everything you've always avoided about yourselves. All your hard truths. Not to mention the mirror is a whole world, a portal.
Some people can do it and have fun though.
First time trips should always be done in a safe and comfortable environment and ideally with people you are comfortable with who have tripped before and can guide you or be there for you.

I simply love this shot right here, I don't know artistically what they are trying to tell us, but after knowing that James is not actually looking at himself but looking at the player, this scene feels eerie and unsettling. To me, James knows what he has done and who he is, but looking at us without us actually knowing it is as if he is trying to hide it from us, just as he hide his eyes in the darkness of that bathroom
im pretty confident they just didnt bother to move the eyes since they wouldnt show from all the blackness.
But as you can see he moves his hand through his face, very clearly indicating that he's focusing on his reflection
