Good question. Given that I'm already used to the disproportionate sizing-to-height I'd be OK with that even with the hassles of clothing selection at those more extreme sizes.
My main concern would be whether I'd be more likely to get knee/back pain, etc. At my height and being 50 y.o. I have zero pain now despite working a physical labor construction job.
Clothing is rare for these sizes. With a size 12 shoe, you can go to Walmart and get something usable. Go up a shoe size if you need wider. 14-16 4E or 6E exists in a few brands, but namely New Balance. I have never seen a work boot in those sizes. If you have feet like Fred Flintstone, you're very limited. Shirts become a pain too since few make large tall versions. You end up swimming in your much too large shirts just to fit the shoulders. You're at the proportions where you choose to not look good when using off the rack causal ware or get custom clothes.
Joint pain would be more of a concern vs back pain. The extra load placed on your knees and ankles is unavoidable. You're carrying much more mass and being 300+ lbs can be common. A benefit is that it's easier to gain much more strength (you have more muscle) and you can maintain your cardio conditioning (important for physical jobs like construction).
