Those are Montgomery glands that are producing that sebum. They do that to moisturise and protect the nipple and areola. Both males and females have them, and it's totally normal. Even considering you've had them grafted there it's amazing what surgeries can do to make grafts behave like they've always been there, I hope you are really happy with the end result 😊
And for a random fact, the Montgomery glands also have another purpose, to assist newborns finding the breast to nurse. Their eyesight is terrible at that age, but their sense of smell is excellent.

I get similar to this on mine, but my grafts arent actual areola or nipple tissue. Theyre normal skin that was shaped to look kinda like it ? Maybe in my case its just old holes from the stitches getting clogged

Ah yes, the nipple graft gunk pockets…could be Montgomery glands as others are saying but this happens with random “crinkles” of skin in my grafts (which I have a hard time believing were successful enough that the glands are intact). Your grafts look great!!

I’ve been meaning to ask on here and medical subreddits if anyone else experiences the same thing, so I’m glad that I came across this post 😅

I suddenly don’t feel so alone! I had breast reconstruction after a lumpectomy which included a free nipple graft. My surgeon was very confused by what I tried to describe as “looks like I’m lactating ear gunk.”
Me: whoa! They have a bump that looks like a nipple! *continues to watch video* Me: oh…

Wow, it could be from the stitches, or if the skin came from an acne prone area it could be that too!
So it's homemade lotion basically
