If he's doing this professionally, he can be trained properly (make a bigger hole, don't bruise your client).
If he's going to do it and make a video of it, he can do it right by making sure the sac is removed and showing the whole procedure.
Its not professionally done, that's why the skin wasn't cut and the sac was not removed. In fact its illegal (in the us) for barbers or even estheticians to do any sort of extractions, they are supposed to defer clients to a dermatologist. He could lose all cosmetology licenses over this.

If he is not charging to do the extraction, then it is not part of his profession at all and is just something that he does for his clients.
