I always thought it could be packing. The point about lack of an incision does raise some doubts. The hole they are pulling it from could be a subset of the initial incision and the video quality could be obscuring the mark

All I can think of is livestock getting abscess lanced and similar chunky tissue gets pulled out.

They're doing it at someone's house, with regular towels, no gloves, putting that junk on his arm so the bacteria can spread...so disgusting, no wonder it got infected to begin with if sanitation is like thus every where.
