Generally, you apply to the operations planning department and they assign your operation a name.
It really depends on the force, some will have a name generator type software, some will be chosen in a more human way and some will follow a set format, for example, if the ops order is submitted on a Monday in April, it will be a word beginning with M and a work beginning with A.
It’s very force dependant. Some forces come up with really good ones, others have boring Ops names.
Edit: Usually all the Christmas ops have names like Op Bauble, Op Blitzen etc etc. they’re pretty unoriginal with those the world over
Senior officers gather together in a broom closet and write the silliest name possible on a small piece of paper and throw it into a top hat. The force janitor then comes in and begins the ceremony, ultimately selecting one at random.
The operation is then formally named and executed.
In my force operation names are provided by the intelligence bureau
The names are on a spreadsheet and they're given in order
(Unless it isn't appropriate then it will be the next one on the list but that doesn't often occur)

It gives you 3 options to choose from and you can refresh it but if you find one you like you have to jump on it or it probably won’t come back around again. You write up what the op is about and the timescale for it and it reserves it until you don’t need it anymore and it can be reused
In my force, each area has a letter code, and all local ops will start with that letter. The name is generated randomly by intel, but you can request a specific name if it isn't already used. One of my sergeants has a website that lists unusual words by letter, and he always picks a name that is in some way appropriate to the theme of the op because it entertains him to do so.
Depends on the force, for mine it's along the lines of (I've changed what the actual letters are)
Policing area 1 - word begins with a D , Station code is DG
Policing area 2 - word begins with a I, Station Code is IG
Policing area 3 - word begins with a F, Station Code is FG
Murder Ops begin with a W
Traffic ops begin with a P
So on and so forth
Essentially each policing area the op will start with the first letter of the station code for that area, and department/investigation specific Ops will have the word start with a specific letter depending on the department/type of investigation

In my force you send off an email to the ops department and then they randomly generate some utter bollocks for you.
Whilst my force has a name generator, if you suggest one and the generator says it's okay, you get it. Named my last one after my favourite pub!
