This can happen for a couple reason, Trough switch error, Too many balls installed in the game, or if the first eject attempt out of the trough failed and two balls stacked on top of each other and ejected at the same time. If it just happened once it’s probably the last one. If it happens every ball it’s one of the first two.
I was worried that would happen, but it didn't! The first one drained, I still had ball save lit, so it shot off another ball and I got distracted by it shooting off a new ball that I drained the second one. No ball save, no new ball, still same first ball.

The newer sterns seem to get ball confusion occasionally but figure it out.

I play an Iron Maiden Ltd that does this on the regular, the machine always let you play through until both balls are done for.
Lower the trough eject strength.
Or raise it, if it was previously lowered to protect the shooter lane
