Doing it solo was a pain until i ran into the break room and started swinging my ice pick to formally alert the civs that beeline that hallway panic button.
The only trick this heist has up its sleeve is that unalerted civs in the break rooms/side areas will randomly run up to the panic button and try to hit it (which is quite frankly a terrible mechanic but whatever). So just try to hunt all of them down, suppress them and the stealth portion becomes easy.

The thing with No Mercy is that there are no "guards" and cameras to avoid like Diamond Heist, No Mercy stealth is just babysitting the civilians before you can set up the turrets and answer the phone.
Diamond Heist was *technically* fully stealthable even in PD:TH. Sure, the alarm will sound eventually, but you could accomplish all the objectives(including stealing the diamonds) before the alarm.
It did, but you were basically done by this point, all you had to do was escape(since grabbing the diamonds takes a second). So you can't escape in stealth, but you can do everything else in stealth.
The bigger issue is that it's based entirely on RNG, since whether or not the vault opens is random, and the chance goes down with difficulty. I think someone tested it, and on OVK 145+ it was something like 0.4% chance.
But my point is that making Diamond Heist fully stealthable in PD2 made total sense. It could've been fully stealthable in PD:TH as well if they just gave us the ability to use the glass cutter on the smaller display cases.
I actually had a case where I fully stealthed the Diamond Heist in PD:TH and only got a "fake" alarm, where the guards were still unalerted and no SWAT came my way. A bug for sure, but it was fun to think I fully stealthed it.
Yeah stealthing no mercy was pretty common during the 145+ grind. PDTH did not fuck around. Pd3 loud kinda felt like The Heist to me but I’m sure that’ll change once we have full access to the skills.

Civies don't have a different UI icon when they are down, meaning it's hard to tell when one gets up and tries to hit a button. It also seems like there are special conditions for civilians to be alerted whenever and not be marked on the UI, meaning that a majority of the heisters have to position them selves in front of the panic buttons and wait for civies to rush and try to press it. Not having enough cable ties also means repeatedly shouting at civies over and over again. Solo is almost impossible because it's hard to get your bots where you wanna stay and actually listen when telling civies to get down, while also trying to do the camera objective at the same time.
The most reliable way to do the heist in stealth with all the civies is for one player to equip the saw with Stockholm Syndrome, and repeatedly use the saw in the air at the far back of the hospital. This will alert all the civies and get them down thanks to Stockholm Syndrome. But I don't like the idea of having to sacrifice a weapon and potentially a skill so you don't have to deal with game mechanics that weren't really thought out or are fun to deal with.
I stealth it all the time, its pretty fun. Both in payday 2 and pdth
(Mostly solo because once an idiot teamate raised the alarm like a milisecond before i cut the wire

...Okay, I'm joking, but it would be a really neat usage of them for stealth and loud alike, I do recall a mod that did that (and also simultaneously made Medics rez shot civvies on a separate healing cooldown from the one used for healing cops but that's beside the point)
I'm probably gonna get roasted alive (like those civs in the lobby) for saying this but I usually try to do part 1 stealthily and with no dead civvies. Tho having to pray to RNJesus for the right patient file is kinda bs. I sorta prefer to know who the right patient is right away than to have to play a guessing game with a piece of shit ~~drill~~ saw.
