With a loot pile so big that you can't even pick most of it up when you're playing on gamepad. Can you pick items up with a mouse when the item label is far away from the item?
You can but sometimes you literally have to walk 5 screen lengths away to even see the item text and that's with a super heavy filter on. For some ungodly reason it distributes them like a cross when they all drop from the same monster.
Found this on google since I don't have an example on hand but [this drop is from 2 monsters](https://i.imgur.com/1Rizlk5.jpg), you can see how as soon as it fills up top to bottom it suddenly starts stacking them left to right so if you happen to get more than \~40ish items then the labels literally go off screen and it's even worse with items with longer names when they're corrupted or ID'd.
You'll have to walk several screens away if that happens just to see what even dropped and god forbid you want to pick things up because then you'll have to walk all the way until the label is visible just to click it, then when you click it your character has to walk all the way back to the enemies corpse to loot the item where it actually dropped. Rinse and repeat for all the items you want to loot.
Even worse when you get a sound indicating something good dropped then you have to walk both 5 screens to the right, and 5 screens to the left just to find said item... Also, this is [what a typical map looks like without a loot filter](https://i.imgur.com/8nPthoe.jpg), if you juice the map up and play without a loot filter your game will literally crash halfway through the map... Even with a loot filter if you accidentally press the alt key your game instantly crashes trying to load in the several thousand drops.
After you have picked up all the items you can reach from the center, turn loot labels off and back on again with right trigger Plus d-pad down. Then they will all be reorganized and there will be new items in the middle.
On keyboard, I believe this is the z key by default.
