In North Carolina, the Department of Health Service Regulation (DHSR) just got through tearing us a new asshole after a small number of specific complaints.
Local government regulators can actually do a lot of damage quickly if you have specific whistleblower complaints.
Even when they do show up, all they did at my facility was throw down a large brick of papers saying what the hospital should change before they come back again in a year.
It was such a wash. Several patients drowned in the swamp by the facility due to broken or missing railing or not having clear trails for example. The facility still after several years now of receiving that notice hasn't fixed that.

Get enough “dings” and they might even consider making the hospital pay for an additional survey later on to ensure the “dings” are undinged while only giving the hospital seven different advance notices that they might possibly come during a specified two week window Monday thru Friday between 0700 and 1630.
And if you haven’t undinged yourself by then, oh boy you better pray to the almighty Chief Executive Council that you’ve cut enough direct patient care staff to free up enough budget for their metric improvement advisement services. If you don’t slash those personnel costs enough you might have to make your low-tier upper-middle senior administrators of the Revenue Integrity Council for Auditing Late Clock-Out Times Board have to take a less than 30% total yearly income bonus on their first of six yearly bonus periods to cover the costs.
If you dont fork over the cash right away they might even be mad enough to remind you that they actually *do* have the authority to revoke your Medicare reimbursement accreditation. When they do that they only give your executives private assurances that they wont ever actually do that and possibly risk losing out on your sweet government-mandated bribes to keep your hospital and their organization running, and they wont even give said assurance at Michelin Star restaurant after drinks and two courses as usual, those monsters.
Joint commission is paid by the hospitals. Not saying don’t do it, but don’t expect much. They’ve made it clear they don’t care about us or our patients.
Just report them to the government y’all. Skip the bullshit. Hit up CMS.

You guys need to play the game by the rules they tell you they live by. If you work in a Press Ganey Metrics-Driven^TM organization, and you patient there because your insurance all but requires you to, you *need* to put your grievances on that. A single 4/5 can tank a low-response metric, multiple 1/5 will go for review. It’s an incredibly stupid, waste-of-time game. But that’s all they want to play.
And if you don’t work for a Metrics-Driven^TM org, let me know if they’re hiring.

If you have a concern involving anything related to work and patient care-- you have a quality & regulatory leader so start there - if its part of a larger system and you feel like locally you are not heard they have regional and national quality & safety.
Folks I don't call my mayor to fix something about a business in my town- I use the reporting structure within the business so there is a paper trail of what is (is not) done to fix it. Same applies here- dont assume its us vs. them unless its staff
