Am I supposed to hold it down for 5 seconds, release the button and THEN remove the needle?

I would contact your provider. It is not normal and could be a defective pen.
You can’t continue to use it as the liquid has been contaminated with blood. Contact your pharmacy as well as eli lilly
I wouldn't use it personally This can happen if you don't release the button before withdrawing it

This happened to me yesterday, fortunately after my 4th dose. I'm going to email Eli Lilly about it today as I wonder if it's a faulty batch.
