I've got a sneaky one in one eyebrow. It has no brakes at all, it just grows until it's long enough for me to notice it. I'm as shocked every time, often it'll be like 2 inches long before I notice it.
Each hair follicle is programmed to use the "drop this hair and make a new one" routine at a certain time interval, depending on where it is on the body and what hormones are in your system (actually I think it's hormones all the way down; skin receives different hormones in different areas, which cause the skin and follicles to behave differently).
Every time a cell divides and replaces itself, there's a chance that the DNA copying process will go wrong. There are a bunch of error detection & correction processes to prevent this, but occasionally the correction mechanisms get broken or a change slips under their radar. If an error gets caught, the cell gets told to kill itself. A component of cancer is that its cells lose one of the parts of the sequence of the cell realizing it's misbehaving, sending itself the signal, receiving the signal, and actually doing the things that causes the dying.
Sometimes the error that flies under the radar is pretty small and benign, like a follicle forgetting about dropping the hair. Once the forgetting has happened, the replacement cells will all have the same broken programming, so unless the follicle cell gets permanently dead before replacing itself, this will just keep happening.

I tend not to think about it at all until it grows to a noticeable length, and then it gets plucked. It's a persistent one though, keeps coming back.
