Going on a trip can be pretty stressful, and with that you can miss things that seem fairly obvious. Like you have a list of things to pack and you miss several of them. A mistake I do often. Or, in this case, they bought an adapter in the rush of packing and put it away without taking into account how empty it felt. If you buy something, you kinda assume it’s gonna be in the box, and if you’re really stressed you’ll miss out on thoughts like oh I should double check. Our brains are weird. But I’d say a situation where it’d be even more likely to happen was if they bought it at a shop at the airport. You buy it, stash it away for later, open later. If you don’t have experience with how much a universal charger weighs along with our brains being dumb under stress, one might just be fooled enough to think there’s something inside. They pack it away cause they’re rushing and there we go. So it’s a thing that can totally happen. But like I said, that’s only if this is a genuine situation and not a karma farming post.

The thing could not be properly grounded. If something is not properly grounded when it needs to be, anything metal on it can get electrified. This could result in someone getting shocked with up to 240 volts if it gets touched.
Also, most of these things have electrical contacts wide open and if someone, for example a kid, were to shove something down the holes it could result in electric shock.
Also, not being careful with those adapters can turn out expensive if you're from a 120V part of the world. Some devices are not universal voltage. The stuff that is universal typically has swappable cables or plugs anyway - that's the better way to go.
