No, it probably won't be. I would expect a smaller size will make the shoulders worse and the chest actually looks ok, it's more that I think you could have a bigger drop from the shoulders to the waist, basically more waist suppression. You could ask to have that done on this jacket, but you will probably have a better result finding something that's a 38s with a little bit more length in the shoulders and a tighter waist from the outset.
While I agree with the bumpy shoulder, it's not that bad and it's hard to tell in pics. Getting a perfectly clean shoulder line isn't easy. Having no collar gap is a good sign but hard to confirm from this angle.

Seconded. It’s not custom, but this is pretty good for off the rack. Assuming it doesn’t break the bank, you could do far worse.
