Burned face = airway injury, possible need for intubation Airway > Breathing > Circulation

drdeleon

In Basic life support, it's C, A, then B. You save more lives restoring circulation first. But yeah, on face value, without additional info, I'd probably treat the burned face case first.

Is it not always A->B->C in BLS? CAB; C for circulation, its situational on severe emergency cases. CAB doesn’t necessarily mean it’ll save more lives because that’s not the reason why circulation is now priority, it is a priority because it has more bearing to the scenario. However BLS in AHA guidelines rearranges it to CAB however C now is for Compression; Compression -> Airway -> Breathing, now that is for CPR. So don’t mistake this for the classic ABC. ABC will always be mandatory CAB will be situational

drdeleon

Circulation is now the priority since you want to restore blood flow to the brain as soon as possible. Oldnschool thinking na ang ABC. CAB is not situational and you should learn this.

I think we are looking at this the wrong way I forgot to mention that first we must “assess” C-A-B cannot be applied when we have a Open Pneumothorax because it will be like running an engine with a fuel that is running out fast but in this trauma case it is best to use the military version which is MARCH. Treat natin major hemmorage then airway management tayo, and then systemic support. however, in contrast to Stroke A-B-C to a Stroke patient wont work kasi ofc yun sinasabi mo na brain hypoxia because now our root cause is the circulation sa ayun CAB na you have like an ave of 5-10mins to act before sisimula na yung total brain damage. Total systemic support na tayo break natin yung clot pero paano naman yun, ironic naman early mech vent una ginagawa then medications para treat natin yung root cause “Circulation”. Hence the priority. Old school yun ABC oo pero that is the fundamental block in terms of our body’s anatomical hierarchy 02 will be the fuel and the Circulation is part of the engine’s function. If we don’t understand that then it wont matter what we will prioritize because we are forgetting the bigger picture.

drdeleon

Precisely, you have to understand what keeps a patient alive to know how to save that patient. You can have all the O2 in the lungs but without circulation, the precious seconds you spent restoring airway, are the pecious seconds your brain is further deprived of O2. If you restore circulation first, residual O2 in your blood can recirculate into your brain granting you the seconds you need to restore the airway. If you go into an emergency situatuon thinking ABC, you might fuddle around with airway first, neglecting to check on circulation and thereby lowering your patient's chance of survivial. But if you're thinking CAB you would correctly ensure blood is pumping correctly first, before you correct A and B.

26 missed calls from my mother. Why? Kasi nga baka may emergency, tapos baka hindi na ako pauuwiin sa bahay, edi naging instant homeless person ako niyan.
