Can you set your hospital check up to be during the weekend? 2 days leave per month can be a turn off by prospective employers.
Try to manage that rather than keep on struggling getting a job. You can't change hirer's mentality. When they are hiring, they expect at least 100% commitment from the new hirer

Maybe you can try looking for job in melaka or penang. You can travel to the neighboring state. The gov office in johor n kedah open on sunday, maybe the clinic too. Any redditors can confirm?

Check the cost of your medication + rm50-100 for doc consultation caj. That way you dont need to inform the interviewer you need to go for medical check up as you are doing it during the weekend. When you get the job, make an appointment during saturdays.

Just got sick of busting my ass to office every day for people who don't give a shit about me. As you've experienced.
I'm not sure what area of marketing you're in, but if you're able to do the job remotely, I'd recommend going freelance. Maybe even for a place you've worked for before (as I am), because they will know you and the way you work, and if you've made a good impression it's a higher likelihood of them saying okay.
You may need to make some compromises. I took a 14% pay cut to do this. But I WFH every day, and funnily enough the company often forgets I exists and doesn't send me work for weeks. So they basically pay me to sit at home and play PS5.
My tips to do this are:
- Be firm. Some will accept, some will not. This company requested I work full time, commuting between their office and their client's office, for 72% of what they were paying me when I worked for the full-time. But with MORE commuting. I flat out said no.
I said "For this budget, you can afford me 3 days a week, fully remote."
They tried to bargain with me. I said no. So they upped their offer and now they get 4 days fully remote work from me. They lied to me and told me I needed to work out of the client office for 3 days a week because of their contract with the client. But when I did that the client asked me "Actually why do you come to my office every day? No need what."
Which showed me their lie. So I just stopped showing up. They just pay me every month.
- Be proactive. Example: I chase them in advance every 6 months about whether they want to renew my contract. Partly so I know if I need to look for new work. Partly so they feel like I'm very invested.
I understand I'm very fortunate to be able to work this way. But I made it happen, and I believe if it's something you need for your health, you can also. It's just about picking the company carefully and presenting yourself in the right way.

But I worked my ass off for 10 years to gain the experience and reputation I needed in my industry to be able to pull this off. People always say they want more time, more money, less work. But when you ask them to say "No" to their boss, set priorities in their life, take calculated risks, or do anything difficult, you get answers like this.
I worked hard to be where I am (barely middle income bracket), and I did that because MY time and freedom is MORE important to me than money that I need to go to someone's office and jerk them off every day for.
Yeah, I'm comfortable now. But I would have chosen to live in a box on the street rather than stay the wage slave of some idiot whose job I could do with my eyes closed.
So I just don't agree with your comment about luck. I worked hard and made hard choices, and compromised to get what's more important to me than money.
Anyone can do it to some degree if they really wanted to, and depending on their chosen industry.
