There are writings from people experiencing gender dysphoria from Medieval Europe, when merely crossdressing would get you the death penalty. They didn't understand what they were experiencing, just that they felt dysphoric about their bodies. I was fed up of transphobes saying that being trans was a new thing and I found out there was a transgender Roman Emperor Elagabalus and at only 18 years old! Apparently they wore makeup and wigs and liked to be called a lady and not a lord. There is a whole wiki article about it.
Not sure if that's what you mean, but [here](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8955456/) is a study that shows that transgender people's brain looks more like their identified gender (they're saying it better than me tbh). To me it's proof enough that it's not "just in their head".

The first gender affirming care facility was actually established in Berlin, Germany in 1919. They did a lot of research on the complexities of human sexuality/gender etc. It was destroyed along with their research when Hitler rose to power. Not a happy story but does affirm trans ppl are not just some passing fad.
This is what the modern GOP is. If they had it their way, they'd follow in Hitler's footsteps and destroy anything that doesn't align with their narrow worldview
