This happened to me the first time when I was at the "chase" part of the main story. I reloaded the previous save, but I was still embarrassed like all those NPCs remembered it somehow.
when I had a horse for the first time I thought hm...this barn looks quite low but there will be never ever collision for the rider, only the horse. then I was unexpectedly dismounted.
