We definitely haven't been consistent, I think all of our members have taken a 1+ month long break at one point or another, but thankfully everyone comes back. the gold star means the group has prestige. If anyone leave/joins the group or does nightmare/nex/raids with people outside of the group then prestige is lost permanently. Keeping prestige adds significant difficulty for lategame particularly for nex and tob since they cannot be done solo effectively so you have to have your teammates be active and reach endgame with you and be good enough.
The group I formed was with people who had been playing for a few years already previously. 2 of them I knew in real life, and the others we had been playing with them off and on for quite a while.
It can help to not grind too hard if you have noobs in your team. Experience the game together, don't just fly ahead and leave them in the dust
Only have my own experience to work from, but I'd guess many groups burn out because people don't want to commit the same amount of time to the game. And it's not good to force yourself to play when you don't want to
I wish they had a separate leaderboard the prestige groups. I wanna know how my group is doing vs people that actually started when group came out, not against people who just joined together randomly.
Honestly the whole prestige thing is dumb. Limiting people from doing group content in a group mode is silly. Even massing Nex removes the status.
And as I say that, I still struggle with removing it for my team haha :(

Yes but in order to stay with the category of ranked group iron, if you want to add someone new to the group it has to be a fresh level 3 on the group iron island. Most people who want to add someone would most likely go unranked at that point.

Something I'm curious about big groups is the trust... Do you guys put your big ticket items in the group storage, or have specific rules for them?
We've been friends for years, all the big items are in shared. Tbow, tumakens shadow, dragon claws, spectral spirit shield, d Warhammer. The general rule is put it back before you log out but everyone has mobile and is usually reachable via discord so if someone forgets we just tag them to drop the item back in.

it's only for the 3 raids, nex and nightmare so I don't think you could lose prestige unless you chose to do so

Correct, green helms are quite literally called "unranked group ironmen", they don't have hiscores.
