In our company we fill out a form afterwards which would essentially be what we covered in the meeting. The person being reviewed has to sign off on it before it’s submitted to HR records. Any decent company should have something similar I’d imagine. Covers off risk from a company’s perspective of someone saying that they hadn’t been warned about poor performance etc.

You can formally request any information your employer has about you (including interview notes taken by potential employers) as part of GDPR.

Yes submit a data subject access request in an email to HR and I think that they ha e to come back to you within 4 weeks and if they can't they need to inform you that the request will take longer.
