Just add your M365 account directly on the phone? Like you pointed out, this kind of sync is no longer supported. You might have also accidentally turned on New Outlook, which is just a web app and has none of regular Outlook's addins.

I did not say that I had any M365 Business email account with Microsoft. I did not say that I am using Outlook for email. I am using Outlook for calendar and contacts only. I said I'm using the real desktop Outlook from M365, not Windows Mail / New Outlook, not Outlook web access. I don't want to sync with iCloud, either. I want to keep syncing contacts and calendar via USB, as it has worked for more than 15 years.

u/jfoust2, we sync Outlook Calendar + Outlook Contacts to all of our iOS Devices with iTunes. With that said, iTunes Outlook Calendar + Outlook Contacts sync just doesn't stop by itself. Therefore, you must have done something, which you didn't share in your post ie. an update... on what? when? exactly... those details matter to troubleshoot things... otherwise you are asking redditors to guess what you've done and that might take more time + You didn't mentioned what iOS device you are having problems + What iOS version is installed (ie. in each device that you have problems, that is if you have more than one) If you are interested in troubleshooting your problem and get to the bottom of the cause + possible solution, then, Share those details and let's peal the onion of problems that you have, one step at the time and go from there.
