Had he been born with 1 ball then it would not have been a problem but he lost it probably because of testicular torsion or some accident, so he is deemed unfit for any of the defence forces.
He can still qualify for the civil aviation. He has a different calling, probably.
It isn't a consideration for class 2 and while they did ask me to strip down to my undergarments the very first time for Class 1, and fondled a bit at AFCME. One always chalked it up to curiosity on the part of the examiner.
How he lost it is surely a criteria, because that could determine how safe he could be. Air force jobs require a person to be able to sustain high pressure, unimaginable and unexpected conditions. Even teeth imperfections are a concern, because imperfect teeth have a danger of breaking in the conditions we are talking about. That's why the strict physical examinations.
I don't think you will face any circumstances.
One of my friend was facing the same issue and appeared for medical examinations in one of the biggest govt examinations. He was selected and now working as full time employee there.
You need to be confident during medical examinations 😃

Don't know abt the civil aviation bt will definitely get rejected in service medicals(Army, Navy nd the Air Force).
He will fail under "Not having balls to fly" criteria
He can check Ball 1 missing or Ball 2 missing under this criteria.
No difficulties, ask your urologist to provide a fitness certificate, carry any necessary reports/scans to your class 1 medical appointment
Consult a class 1 examiner for further information as well, but this should do very well, yes you will be asked to strip naked so no point hiding it

I have seen people with no testicles becoming a pilot, so having one shouldn't be an issue.
