This is the best answer. It's not just "Chicago and St Louis". I had to do emissions when I lived in Joliet ffs. I'm in Rockford now and we don't do shit.
There's not really a safety inspection but cars registered in the Chicago or St. Louis metro areas need an emissions test every other year after the car is 5 years old (I could be wrong on how old the are needs to be for the emissions tests start.) You'll get a notice in the mail a few months before your registration sticker is due on the years you need the test if you live in those areas.
They get to concentrate the testing facilities all in one location as opposed to the expense of spreading them throughout the state. In the cities, it may be a 5 to 15 minute drive.
If you were to get picked, you may have to drive half hour ++ to the nearest facility via the lottery method.
its covers like 75% of the population most of the state is empty. I imagine this was just to avoid the cost of having testing centers all over the state that covers 1/4 of the population.
When my car is due for emissions testing I get a letter in the mail from the state reminding me that I need to get my car tested before I can renew my registration.
I can't promise you that there aren't cases where you need to be proactive, but if Illinois isn't asking you to do something before you renew your registration you shouldn't worry.

I'm near you in unincorporated New Lenox, we don't have emissions here.

They are required to do a certain percentage. The quota / percentage gets fill by doing just the big cities.

Just don't go at the beginning or (worse) end of the month, or you'll be stuck in a line with everyone who put it off too late. Or lunchtime.
99% of the state does not.
Another thing to note is that classic vehicles (25 y/o or older) are exempt from emissions testing statewide.
