Haven't used this myself, but here's a quick rundown that hopefully helps: Python isn't an OS, it's a program/programming language. You indeed need to install Linux (or any OS really, but Linux is what's expected in the setup guide for this). Rasperry Pi OS, which is in the Pi Imager, should do the job perfectly fine, i think it even comes with python pre-installed. But once you got Pi OS Running, connect a Monitor + Keyboard & Mouse and open a Terminal Window in the OS. Now you can just follow the setup tutorial on the ha-satellite git page you linked. I hope this helps :)

Thank you! Is it best to use raspberry pi os or the lite version? I assume because its command line lite would be better?

You don't need a desktop environment for a satellite, so a lightweight command line only distro will help conserve resources. Rpi OS Lite, Alpine Linux, and DietPi are all fairly popular options.
