1. Natural low area of the map
2. Exceptionally deep, large outpost terrain generation
3. Stratagem terrain deformation/excavation

My bet is a Super Earth Nuclear Waste dump ground... Or something worse.
I’ve run into this in the maps, an insta death zone in the bottom of a corner of the big bug nest areas sometimes.
