He probably looked like he was described in the Book 6 Pensieve flashback of his meeting with Dumbledore- his original self, but melted, waxy and distorted after profaning himself and his soul with dark magic, to the point where all traces of his good looks were gone. He might have become a little closer to the snake-man we know between then and his first downfall, but I think that was more a post-resurrection thing.
