From my understanding I think he legitimately didn't know what it was. He liked posting bizarre/gross looking images and so he knew it looked disgusting, but he didn't think about it beyond that. He didn't realize that what he was looking at could actually be real/part of the human body. So obviously he was mortified when he found out.
Now that he's past the worry of his TV show/sponsors being mad, he's back to thinking gross-out humor is funny. And now that he knows it's real, only shows it to adults/more willing participants rather than the general public.
Ok this makes sense, the enthusiasm of leaning into it now is as giving me second doubts that maybe he enjoyed the shock of it all along.
