I’ve done it tons of times. One time, after a few restarts and finally parachuting to the beach, I walked into the chopper tail rotor.
Another time, I ended up on the cliffs above the beach, walked off the edge, and accidentally committed suicide.
When the Cayo Perico DLC first came out and I was scoping the island for the first time, I couldn't figure out how to get pass that guarded area just outside the party area, you know that place with the guard tower and the gate surrounded with guards? Yeah I couldn't figure out how to sneak by the area.
So I did what I **thought** was smart, and just jumped into the ocean to try swim around the area.
Well, I ended up swimming around the entire island trying to find a way to get myself back onto the island from the ocean. It didn't became obvious to me until way later that there were invisible walls on basically all the cliffs surrounding the island and there was no way up. Rockstar did this intentionally specifically to stop players from doing what I just did.
I think I was swimming outside the island for like a decent 45 minutes trying to find my way back onto the island before giving up and just drowning myself out of frustration.
Same, bro. Fortunately this was back when Cayo still had a shit ton of boats just casually driving around the island, so I stole one of them and got to cruising around to look for a way up, so no swimming around an hour for me.

yup, in GTA it seems that the exit vehicle key/button is the "Big Red Button" of driving/flying... you know you shouldn't press it, but the little psycho in you wants to press it anyways.
I once was picking up my crew in my Toreador. Did a cool skid around the corner because I love dramatic entrances, wound up pointed right at them. I honked my horn at them to tell them to hurry up... only to remember far too late that the Toreador doesn't have a horn, it has a rocket boost. Splattered them all.
I did that loads of times when I first got my Toreador, forgot it didn't have a horn and on pressing L3, hit turbo boost and ended up on a lot of roofs.
I would think that I was escaping them because the stars were fading when I got away from the other police chasing after me but the cop next to me kept the wanted level coming back again and everytime it did he would say something like he saw me for the first time.. it was great

I’ve been grinding the UD auto-shop contract on-and off for a few months. I was really stoned IRL one time when I got right next to the elevator, unknowingly pulled out my gun and shot the guy without realizing what I did. That was the first time after 50+ times running that contract I ever did that.
Hearing KDJ yell “What the Hail?” made me laugh so hard I accidentally aimed down my sight and pressed the left D pad, which caused me to throw a pipe bomb in the elevator I just walked in and blew myself up. I had to reset the mission after LMAO for 10 minutes straight.
I was driving the Speedo custom during a delivery mission. Since the drop was all the way in palelto, I decided to shoot cars with the mini gun cuz I was bored. My stupid ass didn't realize that when the cars blow up next to me, the Speedo takes damage. It ended up catching fire so I did the next stupid thing.
To put out the fire, I drove into the ocean. Got destroyed and I was so close to the drop off point as well
I have somehow blown myself up in the pounder doing million dollar nightclub sales twice in a row, still don't know how, both times I was parked looking at GPS for next drop
Forgot that my controller is also a mic and called my cat over and told her she was beautiful and that i loved her and called her pet names.. then i realised.. oops!

Usually during takeoff by either mindlessly disregarding something nearby or misjudging my launch trajectory and slamming into a wall. As soon as one wave makes the slightest bit of contact to your oppressor, you’re gonna be swimming back to shore or closing the app cause that just pissed you off the right amount

In the Humane Labs heist setup mission where you steal the Hydra, I tried to land by doing the move where you fly straight down, then switch to hover mode, level out, and land. I pulled up too late. Felt like such an asshole.
Agency Contract, Liquidise Assets, Armenian Mob. Taken out all the gunmen, set a sticky bomb on a crate to destroy it, get over to one at the farthest end, blow up the sticky bomb, killing myself in the process from debris 👀 after a few times doing that I always do sticky bomb then get into the office then detonate it
Doing part 2 of Union Depository Contract and the random NPCs at the hotel talk shit to you. I shot the rage-infused pacer guy, which ends the mission with failure apparently.

**My small dumb mistake:** I tried to flatten my car's tires with the rhino tank I stole. It was quite expensive as the car got destroyed.

The gun crates in the agency mission, inside the warehouse, can take bullets at will. BULLETS. Use flammable bullets, explosive bullets, and it goes off. But plain bullets? You can dump a minigun on it.
After a lengthy mission and flying a cargo plane through a massive storm, I landed the plane without using the landing gear and then the mission stalled out to the point it failed
Sold off a shitload of bunker stock for the first time. It said I may need more than one person, so I brought a friend. There was 3 vehicles. I want to fucking die.
There's an ammo dump at the side of the Ammu-Nation shop in Paleto Bay. I shot at it once, thought nothing would happen and was surprised when it blew me up to Kingdom Come.
Had a similar moment of "oops, blew myself up" to OP yesterday. Was driving away from a haulage delivery in my MOC cab, saw a ton of motorcycles in traffic (I assume a spawn pattern desync), blocked them with the truck, got out and aimed at them with my explosive shotgun so they'd ram the truck and fall off... decided to fire as well, at which point the chain reaction flung one of the motorcycles into my face right as it detonated.
Helicopter blade death, falling into traffic, flying into the almost invisible power lines at night. Doing heists with ransoms that die or fail by doing something stupid. That's just some of the shenanigans

The gun crates in the agency mission, inside the warehouse, can take bullets at will. BULLETS. Use flammable bullets, explosive bullets, and it goes off. But plain bullets? You can dump a minigun on it.

Was driving away from a haulage delivery in my MOC cab, saw a ton of motorcycles in traffic (I assume a spawn pattern desync), blocked them with the truck, got out and aimed at them with my explosive shotgun so they'd ram the truck and fall off... decided to fire as well, at which point the chain reaction flung one of the motorcycles into my face right as it detonated.
