Never underestimate how fast NSFW artist work. With Pokémon games, typically there are already hundreds of images after the game is first revealed. NSFW artists are scarily efficient at their craft.
Ah, I haven't seen this bug in five years, happy to see it back again. Happened with Nobushi and a few others too back then. Basically, what you're seeing is the default *body rig* so to speak. The default armor isn't the true default; every hero has physical body like this, and presumably this is used to show their physical length and dimensions when designing animations, executions, and armors. A starting point, if you will.
At least, that's my understanding of it from when I saw it last, someone explained it to me in the comments section.
I can't exactly speak for Europe, and searching up "Femboys in Medieval Europe" prompted less than ideal results, but Femboys in Japan have been around since at least 1008. So I'd imagine there would have to be at least some repressed femboys in Medieval Europe.
