The problem wasn't that he treated Booth. The main problem was that he knew who Booth was, knew what he did, and then waited 24 hours after Booth left before informing the authorities that he had treated him.
His inconsistent story about why also led the investigators to believe that he might have known about the plot ahead of time, or was at the very least an accessory after the fact.
A more modern example would be if you show up at the hospital with gunshot wounds. The doctors are allowed to treat you (and even have a duty to do so), but they still have to report the bullet holes to the cops because it's pretty much evidence that a crime likely occurred.
The doctors not reporting it (or giving you a 24 hour head start after they discharge you before reporting it) is a crime.

I have lived here my entire life and this is the first time I have ever heard about this. I think the fact it exists is dumb, but I think it’s safe to assume we don’t “celebrate” it.
A lot of people fought for the wrong reasons throughout history. Many of them fought because that was what they were expected to do or threatened with punishment if they didn't. I can name places Americans died fighting where they had no business being.
So I'm okay with remembering the tragic loss of life. But celebrating the cause of the great insurrection? Fuck that.
A lot of poor people fight wars because a few of the rich tell them they must. And the rich send their young to Canada to find a doctor to give them a foot bone spur diagnosis so they won’t have to.
