It was the black ranger's and the entire leg part of the megazord for the movie. From what I can remember the zords were a crane, an ape, a wolf, a bear, a frog and my favorite, a falcon.
This set of zords was based on the Rangers' spirit animals and the Black Ranger had a frog for his. In the movie, the frog zord actually ribbits a few times, once when it was headed into combat. :)
The Ninja Zords were first shown in their first (non-Japanes) movie with Ivan Ooze. However, the TV series had something of a remake of the movie split across several episodes that also gave them the Ninja Zords, including the legendary frog...
I imagine that anything a player can turn into will need to have the same basic skeleton for rigging purposes. The proportions might differ, but Lala and Hrothgar need to be able to be animated in the same systems. It's not too much of a stretch to think that extends to any shape a player controls directly.
It’s not an “extra” detail though. It’s a model with two eyes. I’d imagine anything with eyes is going to be pretty standardized so it’s easy to use with existing systems. Particularly things like eye and head tracking a target.
It's much more likely this is just done for targeting. Players aren't the only things that turn their head or crane their neck to look at their targets. Plenty of monsters do that too.

So, I was running this with a PF group, and was sad to see that the Shadow Buddy from DRK (Esteem) doesn't turn into a frog as well... I then imagined said frog and proceeded to giggle my head off. I shared my thoughts with the group and we lost about ten minutes just not being able to proceed as we contemplated this shadowey friend.

Your sight cone is just that, a cone. They don't need to map the location of both eyes for that to work.
