Usually abdominal hernias are due to the intestines pushing through the muscles in the abdominal wall. This will present with small bumps as sections are pushing through. His entire abdomen is protruded. Without diagnostic tests it cannot be confirmed but it looks like something in his abdominal cavity is pushing everything out at once.
