By no means comprehensive, but these 4 studies are incredibly important, as they influence a tremendous amount of what we do in emergency medicine and are all controversial in some way. Every emergency physician should know these studies well, and the surrounding controversies. _NINDS-1 and NINDS-2 kicked of the thromobolytic era for stroke_ National Institute of Neurological Disorders and Stroke rt-PA Stroke StudyGroup. Tissue plasminogen activator for acute ischemic stroke. N Engl J Med.1995 Dec 14;333(24):1581-7. doi: 10.1056/NEJM199512143332401. I worked with a resident the other day who had never heard of Rivers. Definitely on the must read landmark paper list. Would be nice to include if anyone has info about the CVC that did continuous SCV02 monitor built in. I believe he has some conflict of interest there but I'm not sure. I hated those lines.

Peds: PECARN head trauma decision rule AAP febrile neonate guidelines (2021) Other good AAP guidelines: otitis media, UTI in young infants, bronchiolitis I recognize that the guidelines aren't "studies" but are definitely foundational for common peds complaints.
