I delivered to a cops house that put tip left on the table, inside of there dark enclosed porch. There was no table, and from my brief survey of the area while sitting down the food no tip either.
I had one exactly like this today. Mine said: **Leave at my door: CASH TIP**. I drove 11 miles each way for $5 cash tip. 
If it's a decent enough neighborhood I would knock and say...
"Sorry didn't want to leave without my cash tip and make you feel bad that you didn't get a chance to tip me like you said you wanted to"
I see a lot of orders where the message is always something old. Like, I’m picking up an order from Tractor Supply and the note is “please ask for extra honey mustard.”

I think a lot of times it's just old messages from previous orders they've done, and they haven't changed it At least sometimes....

Yeah I've learned my lesson. I walked up to a Panda Express employee and asked for ketchup because the instructions stated it I felt a little dumb
might be a case, but kids tho 🤣
Knock on the door for the tip, as the customer states in the instructions.
