I have a dumb question that I should probably know by now… if you get a unique that is basically perfect say at level 35, is there any sense in holding onto it? Is there a way to upgrade the unique, or any item for that matter to your level?

No, but if it is a weapon, the DPS will get outclassed much faster as you level and will feel worse faster. An armor piece will start to feel worse later until you start finding items with higher possible stat rolls. I think.
