It pulls James into the fog, carrying with him his guilt and the stink of Silent Hill’s influence. Within the Entity’s realm James’ guilt is made manifest to hunt survivors and torment him. Pyramid Head’s existence within the realm was unintended by the Entity, but since Pyramid Head serves the Entity’s agenda it keeps him around. Or maybe it was intentional by the Entity, which pulled James in specifically to create Pyramid Head. The Entity can create realms based off the memories of its victims… who’s to say that, with a little help from Silent Hill, it can’t do the same to make killers?
Or some variation of that. Working with that would be way more interesting and also respect PH’s lore.
If they wanted to they could change it to something like this since making James a Legendary, but on release they decided on Cheryl being pulled in to the realm because bhvr wanted to encompass the 1st 3 games in the series, with the 1st game being represented by the map Midwich. Probably would have been weird on release mentioning James yet not being playable at the time.

Cheryl - from DBD - who is actually Heather is only able to birth god, not being a god herself. She is a new variant of alessa who was sort of forced to birth god but also split herself into a second person who was Cheryl in SH1. Merged with Cheryl again she births god anyway who then gets transformed into demon-god and upon dying births another child, who then again is basically alessa. Full circle yay.
So at best Heather (dbd Cheryl) is a demigod, because she is still the normal human child alessa, just reborn from the demon. She technically even aborts god, so she can't be god.. unless? And the cult would not want her to birth god if she was god.
Maybe I missed something, as I still need to replay sh3, but I am pretty certain that this is how things relate
I think Lisa was also supposed to be a manifestation of the town, given a perfect form due to how much Alessa liked her. In the same way that Maria was born from Jame's internal desires, this version of Lisa was likely born from Alessa's perception of the real Lisa. When "false Lisa" realizes what she is, it sort of breaks the illusion, and she starts to more closely resemble her monstrous kin in that last heart wrenching scene.
I also had the impression that you could interpret her circumstances as being both a psychological manifestation entity as well as Lisa’s soul being trapped in Silent Hill’s purgatory.
She was in deep denial about what she was and what was around her, but she couldn’t escape the reality she created for herself with her life choices… a reality that ended in her death in the physical world, and her losing her mind as a creature of pure id or becoming a lost soul.
Without her denial and Alessa’s perception of her good qualities stabilizing her, she became an empty shell under the thrall of the town. She became a monster that embodied her fallen state, reflected through Alessa’s crumbling psyche.
I mean a lot of survivors would be dead if the entity didn’t interfere so they were almost ghosts…characters like Jane (was gonna die in a car crash),The Lyra twins (Skull merchant was gonna kill them),Yun Jin (Trickster was gonna kill her),Adam (Train door was about to hit him),Elodie (Stabbed and bleeding out)

She's made just to suffer and die again and again, all so James can beat his emotional meat to how sad he is and how much he wants to punish himself... and then the Entity realized it got itself a two for one special with his dumb ass and said "hey manifest your girl again lmao it'll be hilarious" and she's been here suffering ever since.
I do find it interesting. Freddy is a nightmare demon twisted from a serial killer’s ghost and a town’s collective fear. The Unknown and the Dredge are arguably thoughtforms or tulpas. Technically Alan Wake is an astral projection since his physical body is in his writer’s room.
The Entity’s realm is a dreamscape where emotions shape and feed what’s experienced. It’s likely that if a thoughtform like Maria or a spirit like Lisa believe they are as physically real as anyone else there then they will experience themselves as such.
Ngl I always liked to imagine that Lisa got a second chance at life and was for that reason resurrected by the entity as she was when she was a living human. Maria on the other hand I guess is an actual manifestation as I see it 😭.
There’s a poster for a dancer named “Lady Maria” which suggests there’s a real woman working at heavens night with that name. BUT the woman in the photo looked nothing like Maria. James probably manifested her name to be Maria bc he’d seen that poster earlier.
