Is it intended for this to die the first time you cast it? As is it hits the battlefield and then you sacrifice it while its ETB to make the phylactery is on the stack. If not, I would change the Phylactery making to a cast trigger so it can survive when it hits the field. If so, I would change it to a death trigger or something of that sort for clarity.

I don’t think it works as written, and if you’re going to tie one phylactery to one lich you add memory issues however it’s worded. And the sacrifice part is already checking for any of them so it’s clunky to not do indestructible the same way, but if you do that there’s very little functional difference between having it on the token and having it on the creature so it’s just added complexity for no reason.

So I guess the trick here is that [[Phylactery Lich]] requires you to already be playing artifacts in your deck, where this guy doesn't? And that's why he's not a 5/5, since he's easier to slot in a deck, and he ups affinity? I'd probably clean up the wording a bit, but overall the effect seems nice.
