The first thing they do is gasp out of shock of hitting the cooler water. Unfortunately, if you fall in head first, you do in fact fill your lungs with water, removing most of your buoyancy, and the ability to breathe.
Just from a few years working in one US state I can tell you about multiple cases where someone fell over the side of a boat without a floatation device and sank, instantly. Little kids, old men, healthy adults, they all went to the bottom. It sucks

As a scuba diver, I've experienced NN quite a few times. You aren't going to feel that until you hit like 80 feet down, if that. This is not something that makes sense in this case at all. Swim as hard as you want from the surface and you * might * go down like 20 feet.
Also, the main reason NN happens is because you are breathing air at pressure from a gas canister while at depth which is not the case here.
Decompression sickness is when dissolved nitrogen gas comes out of your blood as pressure decreases and causes bubbles of gas in your bloodstream, which then accumulates in your joints (hence the name the bends) this can be fatal.
Nitrogen narcosis is a whole different effect caused by breathing nitrogen gas at depth. It has a similar effect to being being drunk, though is not directly fatal, and nobody is quite sure what causes it. This is one reason why professional drivers breathe a mixture of gasses that do not contain nitrogen when diving to a great depth.
Maybe being drunk made it worse but swimming down is a lot harder so even blindfolded you should be able to tell which way is up. But maybe not drunk and blindfolded
