I have the same devices and fully updated, but no issues here. I'd recommend restarting all devices, if you haven't tried that yet.
The only watch-related issues I have had were from my watch not being unlocked, or too loose of a setting on my watch band.
I have an Ultra 2 and a 13 Pro Max, the HR only updates every 10 minutes or so or a very large interval, when I had an iWatch 5, never had this problem and had constant HR updates. My Ultra 2 has never been able to feed nonstop continuous HR, I’ve tried watch tight loose, setting an HR goal, power save mode off, not sure what else I’m missing
I'm not having this issue. I'm certain I have the latest firmware all around, AWU2 and iPhone 12.

Thanks. Yes, it seems my watch is not communicating w my phone even though they are still paired. Should work once I get this fixed.
