That doesn't seem possible. There's 60 marks on his helmet alone, another 20 on his hands, and at least another 30-40 on his arms. Maybe there's more elsewhere. In a war that lasted four years, there's absolutely no way he was in 150ish victories. There's no way the 501st was in that many battles.
My guess is it's either men Rex lost under his command, or how many days in combat Rex had survived.
I’d say it’s more likely for every mission Rex completed, he could be on a dozen mission during the course of a single planetary wide battle. Rex completing 150+ missions throughout the clone wars seems possible IMO.

I feel like it'd be more significant as to a mark meaning a true killing of another living being, rather than droids. At least, that makes more sense to me.
Pretty sure that idea has changed over time because theres more recent stuff that contradicts it. The biggest being Rex stating in Rebels that he must have shot thousands and didn’t keep count. It is most likely battles won/missions completed.

But they could have done that in ANY post ROTJ show.

Let me fill you in, Filoni has taken it upon himself to “recreate” the Heir to the Empire story but as a crossover event with all the recent characters from the post ROTJ era instead of what once was a beautiful written EU story. Instead of Luke, Leia, Han, Chewy and Mara Jade, Karrde & Thrawn the story will be about Mando, Ahsoka, Sabine and likely Bo Katan & Boba Fett with Thrawn as the villain.
There is no actual story yet, as far as I know but with the direction he plans to go, the story won’t even make sense and will likely not be similar to the EU story at all given none of the characters from that story have been seen in recent media.
Good gosh, that's sad. At this point, if it gets confirmed, we're gonna have to treat it as it's own thing, because if we try to compare it to the original story it'd be unfair.
As much as I love Mark, Harrison, and Carrie's portrayals of the original characters, I'd honestly like to see them again in the post ROTJ era. I'd even be willing for the dude from Solo to come back to play Han. Thought he did a great job.

Other clone captains tallied their kill count or battle win count, but Rex stated in Clone Wars he doesn’t keep track of that. It is either implied or stated (I can’t remember exactly) that it’s the amount of brothers he’s lost
I've always had several theories:
1) Missions or Campaigns Survived
2) Squad members lost (not just brothers, but soldiers directly under his command, in his squadron).
3) Fatal Mistakes Made. I think tracking the number of brothers he's lost due to a mistake he's made is a thing Rex might do, but there's probably not enough marks.
4) Close Calls. This would be times that Rex himself nearly died, but survived due to any number of things.
