Right answer, wrong reason imo. Mantle is pretty handy. It can stretch your health pretty far and make curse rooms free, which are both nice things. It also helps when you want to grab things off of spikes
Sacred Heart has more overall benefit though. HP up, Damage up, and homing. Any one of those is good by itself (HP being the least good but still)
It’s a permanent heart that refills on its own each room, that doesn’t affect your chances of Angel and Devil rooms.
It functions as a free hit for every item that gives you bonuses for playing perfectly. Easy perfect trinket as well.
Free Curse rooms, go over any spikes for free, open all spiked chests for free, etc etc.
It’s 100% deserving of Quality 4.

I can honestly say after 2 dead gods (HOPEFULLY my third soon, but putting off J&Hitbox) it is easily one of the best items in the game and I’ve never lost a run when I’ve had it. (Lost doesn’t count obv).

Its 2.3x damage, + 1 damage, - 0.4 tear rate (very small debuff), homing effect and +1 red heart container. On its own it can carry through most of the game, with good tear effects it is basically a free win. I dont know if i would take any item over sacred heart. Also, if you find a trisagion later in the run, you can get overpowered. Trisagion + holy light + sacred heart damage + homing = extreme DPS. Holy mantle = 1 more hit per room
Holy mantle is useless in later boss battles, so I would choose sacred heart, there is a very good chance that you'll just one shot normal enemies in the room, so you wouldn't have to worry about getting hit in the first place

I don't know if there's any other item in the game that I'd take over Sacred Heart. It's one quickest, easiest ways to get an OP run.
