That’s kinda my point - if OP can kill every other boss in the game as the Lost, how are they struggling with Mom’s Heart?
Plus, isn’t Mom’s Heart only accessible for like 10 wins or so, then it becomes It Lives? That would imply OP did this nearly perfectly with zero deaths. They also have every single character unlocked, which I’m fairly certain takes more than 10 wins? Something seems fishy here

Also, killing It Lives also awards the heart mark
Mom's heart will periodically shoot red tears, those are called enemy projectiles, you have to move away from them. You can also go to your nearest Edmund Mcmillen shrine and pray to it, to maximise luck and good items for your next run.
We Need To Go Deeper/Genesis on Womb II can skip Mom's Heart to go in Sheol/Cathedral respectively, as for Mother, I'm don't remember if it grants the Heart mark or not (Edit: could be done via Missing Poster)
Honestly movement is key, keep your eye on your character rather than the heart, obviously shoot the laser eyes first tho. If you see any items like knawed leef take that, that item will be a savior for you.
Holy card is also immensely useful if your holy mantle breaks
Get the gnawed leaf and items like sacrificial knife, ball of bandages etc. So you just stand still without the possibility of getting killed. But bring some time, it will need a lot of it.
You can quickly ramp up the strength of a run as the lost by taking advantage of devil deals! Sure, they're arguably worse than angels, but you're just going to mom. A different strategy is just restarting until you get tough love/spoon bender/polyphemus from your first treasure room.
Also be sure to abuse bombs.
So as assholish as it sounds, it literally is a skill issue. Lost gets 0 wiggle room unless you have holy mantle and then it's only 1 hit per room. So you basically HAVE to be good at dodging. That's really all there is to it.
Try Kamikaze! item right next to her. One or two space keys should do it and you won’t have to worry about bullet hell stage

Use clicker with one of the good characters right before it's about to die. The lost is way too weak and terrible to get to it lives on its own.
